In our 4-bed ICU, there are many reasons for differences in the care delivered (actual) versus planned. The ICU is intended to take care of critical care patients and the staffing was planned that way. In a smaller hospital it is sometimes necessary to take care of medical/surgical patients in the ICU until an appropriate medical/surgical bed can be obtained. While the usual patient care assignment in our ICU is 1 nurse for 2 patients, these medical/surgical patients not as critically ill, so the ICU nurse can care for more of these patients. Also, at other times during the course of a year we lack the patient volume to keep the unit open at all. Consequently, we close the unit approximately 20% of the time as a result of having no patients. All of these factors produced care hours per patient day that are lower than planned.
