Extra hours and/or overtime required due to increased volume up to 52 patients; admissions/discharges/transfers (ADT) high in many 24 hour periods; patient acuity and increased staff requirements; cardiac patients admitted to Spaulding Rehabilitation Hospital (SCC) on post-op day 4-5 requiring telemetry monitoring as well as IV push cardiac medications for arrythmias; staff vacancies; LOAs; 1:1 Patient Safety Assistants (PSAs) and Patient Observers (POs); admitting more medically complex patients that require increased monitoring; changes in CMS regulations for the inpatient rehabilitation hospitals (IRF) and changes in the long-term acute hospitals for admission criteria (LTACH) have made it challenging to keep census at budget as well as accepting the right patients that participate in 3 hrs of therapy a day 5 out of 7 days (180 minute rule.)
